The submission form is a multi-step process where we ask about your organization and your request in detail. Once you have started your Eastman Foundation grant application, you will not be able to save your work before submission.

Please be sure to have the following files ready for upload:
• Form W9 or Equivalent
• 501(c)(3) Designation Letter
• Audited Financial Statement or Annual Report (Optional)
